APPLICATION FOR USE OF CHURCH/SCHOOL FACILITIES

Instructions

  1. Check the Room Reservation System first to see if there may be any potential conflicts with your request.
  2. Submit your request at least 10 days prior to your event.
  3. Kitchens at the schools are not automatically included when use of cafeteria or gym is requested. If meals are to be prepared or any kitchen use is anticipated, you must contact the Facilities Department.
  4. IF CHANGE OR CANCELLATION OF THIS RESERVATION OF FACILITIES BECOMES NECESSARY, PLEASE NOTIFY THE FACILITIES DEPARTMENT IMMEDIATELY (Dick Barry, 614-410-3247).

TIME (s)
Dates & Times (Required) Spell out all the dates and times that you need the room. If you need it for more than one day, put each day on a separate line.
For each day you MUST include
  • The time you need it for setup
  • The time the event actually starts
  • The time cleanup will be finished

Alternately, if the event repeats for several months, delete the example in the box and just explain the repetition. Ex: Basketball practice will be every Tues and Thurs from 4:30 to 6:30 starting Oct 10, 2001 until Jan 31, 2002.

Plans (Required) Briefly describe your event in a way that can be used on the monthly Grace calendar and on the Grace Web Site.
Equip / Food What equipment or food will be brought in?
Food Resp Who will be responsible for food?
Phone Phone number of the person responsible for food.
Cleanup SETUP AND CLEANUP are your responsibility. We appreciate your leaving the facility in the same (if not better) condition than you found it.

TERMS OF AGREEMENT
By Clicking on Send, I agree to the following terms regarding the use of the Grace Brethren Church of Columbus facilities.
  1. An adult sponsor (preferably a staff member of Worthington Christian Schools or Grace Brethren Church) will be present the entire time the facilities are being used. In case of damage or loss to the facilities, the sponsor is responsible to see that reimbursement or reparation is made to the school or church.
  2. No smoking or drinking of alcoholic beverages will be permitted in facilities.
  3. The facility will be left in the same (if not better) condition in which it was found. This includes:
    • Furniture in the proper place.
    • Litter cleaned up.
    • If locker room are used, they will be left clean and mopped dry.
    • If Grace Brethren Church kitchen is used, it will be cleaned before the supervisor of the group leaves the church. Dishes, leftover food, etc., will be removed from the kitchen. I understand that the church is not responsible for these items.
